Seeking applications for Key Workers and Clinical team professionals to support individuals and whanau with tailored interventions and care plans, particularly in relation to housing needs. The role involves close case management, advocacy, and practical support to help clients achieve safe, stable, and sustainable outcomes. Work directly with clients to assess needs and develop individualised support and intervention plans Provide consistent, wraparound support services, with a focus on securing and maintaining housing Liaise with government agencies, NGOs, and service providers to coordinate care and housing solutions Assist clients with navigating social services, benefit systems, health appointments, and tenancy obligations Maintain accurate and up-to-date case notes, documentation, and reporting in line with organisational policies. Monitor and review client progress, making adjustments to support plans as necessary. Provide crisis intervention and conflict resolution support where needed. Attend regular team meetings and professional development sessions

Job Requirements

Experience Requirements: Demonstrated knowledge and/or experience in the housing or social services sector Experience in case management or frontline community support work is advantageous Qualifications/Certificates: Bachelor’s degree in Social Work, Community Development, Psychology, or related field for Clinical Staff Preferred Health and Wellbeing Certificate for Key Workers Driver Licence: Full New Zealand Driver Licence (essential) Pre-employment drug testing is mandatory Criminal convictions will be considered on a case-by-case basis depending on the nature of the offence and relevance to the role
