The Kaitakawaenga will work alongside whanau to develop and implement whanau aspirational plans within a community setting. This position will provide whanau centred services working towards whanau self-management. It will provide a focal point for gathering health information, liaison, communication, advocacy and other support as identified by whanau. This positon will include assessments, planning, co-ordinating community linkages and education, to enhance people’s skills and experience, enabling self-management of their health and wellbeing. This role should reflect the importance of working with all whanau members of providing advocacy and support in accessing services and helping whanau learn new skills so they can transition from dependency to Tino rangatiratanga.
Service Delivery: • Respond to new referrals ensuring a timely contact with individual and/or family and appropriate caseload management
• Utilising strengths based and holistic approach to service delivery, undertake comprehensive personal planning with the individual and/or family by:
• Work collaboratively with individuals, families, and significant others, relevant service providers and professionals to develop an effective personal plan that assists in addressing a wide range of health and wellbeing issues.
• Navigate service provision through referral and connecting individuals and families to appropriate support services, ongoing development of health and wellbeing, related knowledge and support networks.
• Review and evaluation of personal plan progress and intervention effectiveness with individuals, families and significant others, relevant service providers and professionals
• Attend meetings, forums and training opportunities tin order to be responsive to emerging needs, identify service delivery gaps and ensure individuals, families all receive the most up to date information
• Develop knowledge of community services and resources in order to assist individual, families to access them
• Engagement of 40 identified individual and/or family to complete an annual target Community Liaison: • Build and maintain relationships with other agencies, community groups, Maori networks and relevant individuals
• Attend interagency meetings and work collaboratively with people in the community
• Provide forums, seminars, workshops, newsletter on health & disability issues to keep community agencies and individuals and/or families up to date with relevant information in the field of disability or both local Council or Government changes that may have an ongoing effect on people’s lives
• Promote the organisation at every opportunity and present up to date promotional fliers and pamphlets etc
• Promote teamwork within the community by being a positive, active contributing member whenever possible

Job Requirements

Reporting Health & Safety Communications & team work Working towards a degree or qualification in the field of health, social science or humanities
Knowledge of Te Tiriti o Waitangi, Tikanga Maori, and the ability to uphold Kaupapa Maori in role
Full Clean Drivers Licence
Commitment to ongoing training and cultural competency training
No conviction history (driving offences can be discussed)
