About the Role: The Community Support Worker provides practical, culturally appropriate, and recovery-focused support to individuals experiencing mental health and addiction challenges. Working closely with service users, their families, clinicians, and community networks, this role supports individuals to achieve their personal goals, strengthen wellbeing, and actively participate in their communities. The role requires strong relationship-building skills, cultural competence, advocacy capability, and the ability to work collaboratively within a multidisciplinary team.
Responsibilities include but not limited to : • Manage and follow up on service referrals in a timely manner. • Maintain regular and proactive contact with service users. • Conduct cultural assessments and support service users with goal setting and recovery planning. • Plan and coordinate activities aligned with service users’ recovery goals. • Facilitate both individual and group-based activities. • Advocate for service users and support them in navigating services. • Participate in family meetings and liaise effectively with clinicians and other professionals. • Maintain accurate, confidential, and timely documentation and case notes. • Support service users to access community resources, social services, and relevant programmes. • Promote culturally safe and inclusive practices in all aspects of work.

Job Requirements
Skills and Experience: To be successful in this role, you will ideally have:
• 6 months to 2 years’ experience in community support, mental health, addiction services, or social services. • Minimum secondary school education and/or relevant certificates in health or social services. • Experience working with Pacific communities or culturally diverse groups (preferred). • Strong communication and interpersonal skills. • Ability to work both independently and collaboratively within a multidisciplinary team. • Good organisational skills and the ability to maintain accurate documentation.
Essential Requirements • Fluency in at least one Pacific language. • Full New Zealand Class 1 Driver Licence. • Passenger (P) Endorsement. • Ability to pass a Ministry of Justice criminal record check.
