Keyworker

Rotating roster covering Monday, Thursday, Friday, Saturday and Sunday Shifts: 6:00am to 2:30pm, 9:30am to 6:00pm, 1:30pm to 10:00pm

About the Role The Keyworker provides client-led case management and supports clients on their journey into permanent accommodation. You will develop individual transition plans, build life skills, and support long-term tenancy sustainment within a supportive, values-led environment.

Key Duties • Deliver client-led case management on a 12+12 support model • Manage referral and intake processes • Support clients to build confidence and life skills • Develop transition plans into community or social housing • Conduct room inspections to support tenancy sustainment

Must have at least 1 year experience in a care and support environment Must hold a qualification in Health Science, Social Work, Psychology, Mental Health, Addictions, or similar Strong needs assessment and support planning skills Understanding of mental health, addiction, poverty and homelessness Full clean Class 1 New Zealand drivers licence + reliable transport
