Our employer are excited to announce two fixed term opportunities for passionate and dedicated Kaitiaki Hauora (Health Support Specialists) to join their frontline Tiriwa team from 1 February until 30 June. This is your chance to make a meaningful impact contributing directly to the wellbeing of whanau Maori by supporting them to navigate hauora pathways and achieve their aspirations. The key duties of this role will be: Conduct hauora assessments informed by Maori health frameworks to understand and support whanau needs Supporting whanau transitioning from hospital discharge to community care, ensuring access to services and resources tailored to their recovery Work closely with individuals and whanau to navigate the healthcare system and access appropriate services Provide wrap-around support, collaborating with other sectors to ensure holistic care based on whanau-identified needs Act as a key point of contact for stakeholder engagement, sharing information about services to strengthen connections across the community Maintain accurate record-keeping and reporting to ensure accountability and track outcomes Support the planning and delivery of programmes and events, contributing to their smooth operation and impact

Job Requirements

We ask candidates to possess the following: A strong understanding of Maori culture and a commitment to supporting whanau in a culturally responsive manner Proficiency in Te Reo Maori, including good pronunciation, and a commitment to continual learning Knowledge and experience in applying Maori health frameworks within hauora assessments and community work Community connections and networks that enable you to build and maintain relationships with whanau, stakeholders, and other service providers Experience or knowledge in hauora or related fields, with a focus on community and whanau wellbeing Excellent communication and advocacy skills, enabling you to build trust and rapport with whanau and stakeholders Competence in technology and data management, including Microsoft Office A collaborative, empathetic approach with a focus on delivery high-quality outcomes

Candidates must hold a Full drivers licence Pre-employment drug testing will not be required
